I thought this part of the book was pretty confusing, mostly because of the flashbacks. I also thought the part about the guy who actually turns out to be a girl is pretty weird, but I guess it fits the theme of Jacob trying to find out more about himself.
The part where Geertrui chose to stay with Jacob was touching because this proves that she loves him and is taking a risk by staying with him. I predict that Jacob will end up being straight even though the book leads us to believe he is homosexual.
